How To Use the Eligibility Questionnaire

Welcome to the how-to guide on completing JOIN’s Eligibility Questionnaire.

When first entering the questionnaire you will find a list of questions with radial buttons, text boxes, or drop down menus:



To begin, simply answer the questions to the best of your knowledge using the radial buttons, drop downs, or text boxes. Remember the more accurate the answers, the more accurate the results will be. Once you have completed the first page click continue. At this point, depending on the answers you provided, you will either be given a results page, informing you of state programs you may be eligible for, or you may be asked more questions:



Again, complete these questions as accurately as possible. If at any time you feel you have made a mistake or need to change any answers simply click "Previous Page' or just start over. Once you have completed all questions you will be presented a results page:



Click on the services to leave the JOIN site and go directly to the provider of the service that you are interested in. Once there you will get a more detailed description of the service as well as the full eligibility requirements and how to apply for them.

Information and services change often, so feel free to fill out the questionnaire as often as you like.
